If you or someone you know is a St. Louis city resident who has been enrolled in the St. Louis Regional Center and is receiving service coordination, you are eligible to receive services through the St. Louis Office for DD Resources as an alternative to receiving the service from the St. Louis Regional Center.

Targeted Case Management is a long-term case management program provided to eligible individuals age three and above. Service Advocates in the Targeted Case Management program at the St. Louis Office for DD Resources meet with individuals on a regular basis to assist with the goals outlined in the individual’s Person Centered Plan.

Targeted Case Management provides referrals to community resources including housing assistance, food pantries, social activities, transportation, employment, related services. Targeted Case Management can also help with money management and parenting skills.

The Targeted Case Management Program was established to give city residents a choice for their case management services.
