Project CaseFind
Project CaseFind is an outreach effort to identify St. Louis City residents who have a developmental disability and are not registered with the St. Louis Regional Office. To qualify for assistance, a person must live in the city of St. Louis and must have an eligible developmental disability.
Eligible persons are linked to the Department of Mental Health/St. Louis Regional Office for ongoing services. The primary goal of Project CaseFind is to make sure individuals are connected with ongoing long-term case management (Service Advocacy).
During the process, Project CaseFind Coordinators act as short term case managers to assist individuals with support services while they are getting enrolled into the St. Louis Regional Office.
Project CaseFind Coordinators will:
-Link individuals to community-based services such as housing resources, food pantries, transportation, utility assistance, clothing, etc.
-Provide information and referrals to support groups, healthcare facilities, shelters, etc.
-Provide information and in-service training to community groups and agencies
-Educate clients and their families on disability rights and laws
-Attend and support client to initial visits for public assistance (i.e. food stamps, TANF, SSI)
-Assist clients and their families with the enrollment process at the St. Louis Regional office for ongoing case management services
-Link individuals to St. Louis Office for Developmental Disability Resources programs (i.e. Targeted Case Management, Educational Coach, Voucher Services)
