Targeted Case Management
Targeted Case Management is an ongoing service that provides support to individuals who have developmental disabilities. Individuals who are registered with the St. Louis Regional Office can choose to receive Targeted Case Management from either St. Louis Regional Office or DD Resources.
Individuals are assisted with identifying their needs. Case managers then link them to the appropriate community based services.
To be eligible for services through DD Resources, an individual must be registered with the Department of Mental Health/St. Louis Regional Office and they must be a resident of the city of St. Louis.
Services consist of, but are not limited to, the following:
-assistance in completing entitlement applications and/or referrals to community resources such as food pantries, housing, transportation and utility assistance.
